Objective:To investigate the efficacy and safety of sildenafil (Viagra) as a treatment for erectile dysfunction (ED) in patients who have not responded to standard ED treatment. We assessed the safety and tolerability of sildenafil in 10 patients who were not receiving standard ED treatment and who were not responding to sildenafil, to sildenafil (Viagra) or to other oral phosphodiesterase type 5 (PDE5) inhibitor therapies.
Design:A retrospective cross-over study of sildenafil and other oral phosphodiesterase type 5 (PDE5) inhibitor therapies in 10 patients who were not responding to standard ED treatment and who were not responding to sildenafil.
Setting:Erectile dysfunction (ED) clinics. Patients were recruited from outpatient clinics that offer ED treatment. They were evaluated for eligibility and the eligibility criteria of the study were as follows: 1) exclusion criteria; 2) exclusion criteria in the patient population; 3) exclusion criteria in the ED treatment group; and 4) exclusion criteria in the PDE5 inhibitor group. Efficacy was assessed using the Clinical Global Impression Scale (CGI-S) and the International Index of Erectile Function (IIEF-5) as secondary outcome measures.
Main outcome measures:The IIEF-5 and CGI-S scores were measured in patients who were not responding to sildenafil and/or sildenafil and who were not responding to sildenafil or other PDE5 inhibitor therapies.
Results:Of the 10 patients who were not responding to sildenafil, 8 were excluded because of a baseline IIEF score <30, 2 were excluded because of a baseline CGI-S score of <30, and 3 were excluded because of a baseline IIEF score of <30.
Conclusions:In the 10 patients who were not responding to sildenafil and/or sildenafil and not responding to sildenafil or other PDE5 inhibitor therapies, sildenafil and other oral phosphodiesterase type 5 (PDE5) inhibitor therapies were effective in achieving and maintaining erectile function, with no difference in adverse events. However, PDE5 inhibitors may not be suitable for all patients.

A new study shows that the popular erectile dysfunction drug Viagra may be just as effective as a lower-priced version, as the popular Cialis and Levitra.
The findings, published in theArchives of Internal Medicine, appear in a journal journal of the American College of Cardiology, appear to be backed by research from a team led by Dr. David Heyd, a urologist at the University of Texas Southwestern Medical Center, Dallas, in the late 1990s. The study, which involved 18,000 men aged 40 to 75, was designed to test the drugs' effectiveness and found that they helped men to achieve and maintain an erection for more than three minutes at a time. The researchers compared the performance of the drugs with those of other ED medications, including the erectile dysfunction drugs Cialis and Levitra. "The results showed that both drugs had some benefit over Viagra in treating erectile dysfunction," Dr. Heyd said. "It's interesting to see that Viagra did more than Cialis and Levitra in treating ED."
The new study is the first to examine the effect of Viagra on the blood vessels of the penis, using a randomized, double-blinded placebo-controlled, controlled, multicenter study.
Viagra is a popular and well-known drug used for the treatment of erectile dysfunction, but researchers wanted to do something to test whether it was safe for men taking other erectile dysfunction medications.
Dr. Heyd and his team were able to demonstrate that the drug, sold as Cialis and Levitra, helped men achieve and maintain an erection, even though they did not have the blood flow needed to achieve that effect.
But some of the side effects of the erectile dysfunction drugs were too serious for many men to handle and that prompted their doctors to prescribe the other ED medications in response.
In addition, the study found that Viagra was more effective than Cialis and Levitra in treating the underlying cause of ED, but that the drug was also less effective in treating the underlying vascular issue.
The results also suggest that the effect of Viagra was not as large as the effects of Cialis or Levitra, which are similar to Cialis, but was greater for men who were taking the drugs as opposed to the drugs in the study.
The researchers also found that men who took the drugs for more than four hours were less likely to have an erection for the four hours following the study than those who were not taking the drugs as such.
Heyd noted that Viagra has been around for over 20 years, and he is familiar with the drugs and their side effects. "In the past, Viagra was used in the medical treatment of men with ED. It's now approved by the Food and Drug Administration and prescribed to patients," he said.
